Job Summary: The Licensed Vocational Nurse under the supervision of the Registered Nurse, is responsible for the implementation of the Plan of Care by administering specific nursing care to patients in the home setting. Justice: Demonstrates competence in community orientation and stewardship.Essential Functions: • Assists the RN with implementation of the Plan of Care by performing and documenting nursing interventions including patient response, patient education and safety interventions.• Completes a basic assessment within scope of practice, and reports objective findings to the RN Case Manager or physician.• Reports effectively to the RN and documents changes in patients’ needs/problems. Communicates with patient, families and physicians.• Documents patient status in an accurate, timely and concise manner by use of Electronic Health Record (EHR) in-patient home at the time of visit. Documentation is completed and synchronized by 10:00 am the following day.• Demonstrates knowledge and competency in performing baseline-nursing skills as outlined in Agency and clinical policy. • Maintains clinical competencies according to home health care standards. • Reports any signs of treatment complications, adverse medication reactions or changes in patient condition to the RN, Clinical Manager or Physician.• Administers medications and treatments according to Agency policy, accepted standards of care and home health care guidelines, following Physician orders. • Practices infection control and universal precautions for self and other inpatient care activities. Additional Responsibilities: • Maintains patient rights and dignity.• Participates in care conferences, team meetings, team huddles and in-services.• Meets on-call, Holiday, and weekend rotation as required and based on business need.• Meets the productivity standards of the organization• Assumes responsibility for reporting incidents and complaints according to Agency policy.• Adheres to California Licensing Regulations for Home Health and performs duties within the LVN scope of practice.Knowledge/Skills/Abilities:• Possesses knowledge of Medicare/Medical and insurance standards to ensure compliance to regulations.• Effective communication skills (verbal and written)• Flexibility to adapt to new and changing situations; participates as a team player to support your team and Agency needs. Information Management: • Follow and maintain HIPAA standardsRisk Management: • Cooperates in risk management activities and investigations. Safety Requirements: • Is knowledgeable of, observes and complies with Home Health Network Codes of Safe Practice, safety policies and emergency procedures.Additional Requirements:• Participate in weekend, on-call and holiday rotation.Minimum Position Qualifications: Education: Completes necessary education to attain LVN Licensure in the state of California. Experience: One year of Licensed Vocational Nurse experience. License/Certification: Current California LVN license, CA Driver’s License and proof of auto liability insurance; and BLS. Preferred Position Qualifications: Experience: One year Home Health LVN experienceLicense/Certification: Phlebotomy and/or IV CertificationEnvironmental Conditions: • Knowledge of proper procedures for handling exposure to communicable diseases and blood borne pathogens.• Knowledge of the proper technique to handle exposure to hazardous material.Physical Requirements: Defined on the attached Physical Demand Analysis.
